Accessibility & Language Assistance
We are committed to providing meaningful access to all patients. To support that, Mirror Health & Wellness offers the following free services through internal processes and approved federal, state, and nonprofit resources:
Access to qualified spoken language interpreters for patients with limited English proficiency (LEP), using free state-supported language lines or community resources
Access to video relay or relay phone services (e.g., 711) for Deaf or hard-of-hearing patients
Written information available in large print or accessible digital formats upon request
Access to translated patient education materials and forms when available from trusted sources (e.g., HHS, CDC, state agencies)

How to File a Complaint or Report a Concern
If you believe that Mirror Health & Wellness has failed to provide the services described in this notice or has discriminated in another way based on race, color, national origin, ancestry, primary language, ethnicity, age, disability, sex, gender identity, sexual orientation, or any other protected characteristic, you can file a grievance with:

You may file a grievance in person, by mail, fax, or email. If you need help filing a grievance, our Chief Legal & Compliance Officer is available to assist you. Grievances will be reviewed promptly in accordance with our internal civil rights compliance procedures.

You must file your complaint within 180 days of the date of the alleged discrimination, though extensions may be granted for good cause.

Mirror Health & Wellness utilizes California Relay Services (Dial 711), available Video Relay Service (VRS) platforms, and approved free state language lines, nonprofit interpretation programs, or local community health resources when available. We also provide translated materials and accessibility modifications to the extent feasible and appropriate under law.
